EOA Head Start/ Early Head Start 

Please fill in the below information and click on the "Submit" button. Filling out a pre-enrollment application does not guarantee you a spot in the EOA Head Start program. One of our staff members will be contacting you to follow up on your application and other requirements for enrollment.

The following documents are needed to complete a child's application:

• Birth Certificate or Proof of Birth
• Immunization Record (UP TO DATE)
• TB Test (if born outside the United States)
• Proof of income for the previous tax year or   the last 12 months for parent/legal guardian and ALL related household members supported by the parent/legal guardian.  (This includes child support, SSI, TEA, and student financial aid)
• Divorce Decree/Custody Documentation
• Residency Questionnaire (to be completed at a Head Start center)

  PREGNANT WOMEN:

• TB Test (if born outside the United States)
