- EOA Head Start/Early Head Start accepts applications throughout the year. There must be a completed application on file for a child to be considered for any opening. To submit an application, parents, guardians or pregnant mothers must visit a local Head Start location in Washington County or Central Office.
- County (see CENTERS tab). The following items must be provided to initiate an application:
- 1. Proof of Birth: State certified birth certificate, hospital
- souvenir birth certificate, or birth card
- 2. Up-to-date immunization record
- 3. TB skin test results if child is over 12 months of age
- 4. Proof of family's income for the previous 12 months or
previous tax year (income tax return)
